Community Care Team

A need identified in our 2015 Community Needs Assessment & priority in our 2015 Community Health Improvement Plan was better access and coordination of services in order to 1) improve health outcomes; 2) address inequality in outcomes across social, cultural, and economic groups; and 3) reduce over-reliance on emergency services (hosptial emergency department and emergency medical services) by vulnerable populations.

Based on a model in operation in Middlesex, Connecticut (Liberty Bank Foundation, "Middlesex Hospital Community Care Teams, K. Irwin & B. Branch, December 2015), the Flint HIlls Community Care Team (FHCCT) was established in October 2018.

The FHCCT is a working group comprised of decision-making representatives from agencies and organizations that serve the most vulnerable members of our community. The following core principles guide our work:

Principles

• Objective: To provide patient-centered care and improve outcomes by developing wrap-around services through multi-agency partnerships and care planning

• Core belief: Community collaboration is necessary to improve health outcomes

• Core understanding: Psychosocial problems are community problems. No one entity alone can effectively improve outcomes for vulnerable populations

At present, there are moare than two dozen local and state agencies actively participating in the FHCCT, representing practically every service or linkage to service in Manhattan/Riley County.  The participating agencies are:
